2016-2017 New Student Registration

Welcome to Upper Arlington Schools! There are four steps to our registration process for the current school year. (Please note that kindergarten registration for the 2017-2018 school year will begin March 1.)

Please read all of these steps before completing your online registration. Once your online registration is completed, please set up an appointment as listed in Step 3 to finalize your registration.

1. Determine Eligibility

(Approximately 5 minutes)

Please verify your home address to confirm eligibility to attend Upper Arlington Schools.  Please check our District Map & Boundaries to verify your address.

If you have any questions about your eligibility based on your address, please call (614) 487-5000.

2. Complete Online Registration

(Approximately 30 minutes)

Please complete the online form. You'll find the link at the bottom of this list.

Before you begin, gather the following information:

  • Previous school information, which includes the name, address and phone number of the last school (including preschool) your student attended.
  • The names and contact numbers of two emergency contacts other than parent(s)/guardian(s) — trusted local adults the school can contact if the parent(s)/guardian(s) cannot be reached.
  • Your student's detailed health information to be provided confidentially to the school nurse.
  • If applicable, custody information regarding your student. Supporting documentation will be collected later in the enrollment process.

3. Schedule Appointment & Verify Documents

(Approximately 20 minutes)

Contact Dawn Kirkbride at 614-487-5220 or dkirkbride@uaschools.org to schedule an appointment and finalize your registration process. Meetings will take place at the UA Schools Graf Center, 2020 Builders Place.

You are required to provide the following documents:

  1. Proof of Residency — please provide one of the following:
    • Current mortgage statement with the name(s) of parent(s)/guardian(s) listed.
    • Summary page from the Franklin County Auditor's Office website (click here).
    • Current, Signed Rental/Lease Agreement with name(s) of parent(s)/guardian(s) and ALL residents listed including students/children's names.
    • Purchase/Contract Agreement to buy/lease property (if within 90 days of enrollment). Final papers must be provided to the school building when the purchase is completed.
    • Residency Affidavit form 511f2b (click here) if you are living with a relative who resides within the district. The homeowner must provide proof of residency.
    • Current Voter Registration Card.
    • Current Tax Statement.

2. Certification of Birth. In accordance with Ohio Revised Code 3313.672, parent(s)/guardian(s) must supply one of the following documents:

    • Birth certificate, birth affidavit, or attested transcript of the certificate of birth;
    • Passport; or
    • Baptismal certificate or other religious record showing the date and place of birth of the child.

3. Proof of Immunization. Please have your physician complete this Immunization Report or contact your physician’s office for a copy of your child's immunization record. You must bring this when registering your student, even if it is not complete. Please see the following documents for more information:

4. Custody Papers, if applicable. In accordance with Ohio Revised Code 3313.67.2 and the Missing Children’s Act, the parent/guardian must provide a certified copy of the divorce decree and parenting plan allocating parental rights and responsibilities and/or other documents allocating custody or guardianship.

5. Other documents, if applicable:

    • Individualized Education Program (IEP)
    • Multifactored Evaluation (MFE)
    • Evaluation Team Report (ETR)
    • Section 504 Plan

Please note: Registration cannot be completed and your student may not begin school without the above documents.

 4. Schedule a Meeting at the School Building

(Approximately 30 minutes)

Once your registration is completed, a representative from your student's school will contact you to schedule a meeting. Elementary students' meetings will take place on the first day they attend school. Middle and high school students will meet one day prior to attending school to review their previous coursework, grades, and transcripts, and create a schedule.
