This
annual form is required by state law for each child.
Section one provides emergency medical information
about the individual child; section two provides
the parent/guardian with the option to grant or
to refuse to grant consent for the individual child
to receive emergency medical treatment.
This
annual form — in accordance with Board of Education
policy 7540.03 — assure that the child will
use and care for school district technology only
in appropriate ways. The parent/guardian and the
child sign the form to indicate that they are familiar
with this Board policy; the policy is attached.
This
annual form – in accordance with Board of Education's
"Instructional Program" policy –
is for the parent/guardian to grant or withhold
permission for the Upper Arlington City School District
to release/distribute certain information and such
about the individual child covering five separate
areas.
