Transcripts
GRADUATES OR FORMER STUDENTS
Transcripts for graduates (or students no longer enrolled) at the high school should be requested from the Records Office which is located in the Counseling Center at the high school. Please contact Cheryl Fackelman, Records Secretary at cfackelman@uaschools.org or by phone during regular hours (Monday through Friday, 7:30 a.m. till 4:00 p.m.) at 614-487-5240 extension 208 (FAX: 614-487-5289). When graduate transcripts are requested the following information must be supplied:
- Name of graduate
- Year of graduation
- $5.00 Transcript Fee (check, money order or cash)
- Name and address (including Zip Code) of institution where the transcript is to be sent.
- Your signature authorizing approval.

CURRENTLY ENROLLED STUDENTS
Transcripts for currently enrolled students can be obtained from the College Center. Please pick up forms for each transcript that is requested, complete them and return them with signatures and a $5.00 transcript fee to the College Center. We cannot attach a form here as they are color coded each year.
