2017-2018 Back-to-School Forms for Returning Students
Each year, Upper Arlington Schools asks the parents and guardians of returning students to verify and/or update important information such as medical history and emergency contact numbers. These back-to-school forms are completed online and take approximately 15 minutes.
Families should have received emails containing secure links to their students’ back-to-school forms in mid-May. Only one submission will be accepted per student. Once one parent begins the back-to-school forms process for a student, that parent will need to complete the process for that student.
Please complete the forms by Thursday, June 15 so our schools can use the updated information to better plan for the 2017-2018 school year.
If you have not received an email containing a secure link to your student’s forms, please notify the district by emailing firstname.lastname@example.org.
Bus routes for the 2017-2018 school year will be updated in August 2017. An email will be sent to families when information is available. For more information about busing, please visit our Transportation Department page.
