FORMS

Hastings Athlete Eligibility Form Required by Hastings only.

OHSAA Authorization Form (HIPPA—Health Insurance Portability and Accountability Act of 1996)
Must be filled out by a student and his or her parent/guardian.

OHSAA Preparticipation Physical Evaluation
Must be filled out and signed by both the parent/guardian and a consulting physician.

Emergency Medical Information/Authorization
State law requires this annual form for each child. Section one provides emergency medical information about the individual child;
section two provides the parent/guardian with the option to grant or to refuse to grant consent for the individual child to receive emergency medical treatment.

Parental Consent for a School Sponsored Field Trip
Upper Arlington Schools' form giving consent for a school sponsored trip.

Parent Transportation for Field Trips & Co-Curricular Activities
A parental agreement indicating they meet the qualifications to provide transportation for student athletes.

Transporting Students in Private Vehicles
A parental agreement granting permission for their student to ride in a parent driven, private vehicle.
